ACT Community Pharmacy Transformation Educators Badge Application

The ACT Pharmacy Collaborative and AACP are launching Cohort 2 of the Community Pharmacy Practice Transformation Educators Badge Program. This program aims to provide a fun, contemporary way to connect community pharmacy practice leaders and educators, ultimately leading to recognition of educators with proven competencies in community-based pharmacy practice transformation.

To apply for this program, please complete and submit the form below. Once the application period ends on Dec. 13th, your application will be reviewed for eligibility and a decision email will be sent to you before the end of the year (2024). 

Please note: If accepted into the program, a required fee of $149 will be charged.

I understand that if I am accepted into the program, I am agreeing to complete the ACT Pharmacy Collaborative Community Pharmacy Transformation Educators Badge. I am agreeing to complete all requirements necessary to achieve the Badge. This includes attending 4 of the ACT Webinars. I also understand that failure to complete requirements by the established deadline in Summer 2025 will result in not achieving the Badge.
